Case management - support and stabilization of the employment relationship
Extended periods of illness are often accompanied by uncertainty as to whether one can continue their previous job for health reasons. This situation is equally unsatisfactory for both employees and employers.
Our case management (CM) service specifically addresses this point of uncertainty and disorientation. We facilitate communication between all parties involved to maintain or fully restore the affected employee's ability to work. Through close coordination between the employee, the company, and the rehabilitation provider, we examine whether and how the employee and the company can be reunited.
Often, simply adapting the workplace with appropriate aids is sufficient. In many other cases, valuable, person-specific experience and skills can be retained within the company through internal transfer and made available elsewhere.
Together, we identify new and innovative solution strategies as needed and initiate the necessary processes. This includes establishing contact with other institutions, such as the advisory services of the integration offices or the employment agency, and providing support in finding suitable jobs.
Target audience
This offer is aimed at employees receiving sick pay. Participation requires approval from the responsible rehabilitation funding agency.
Goals
The aim of the offer is the sustainable integration of employees with health impairments into companies in order to prevent unemployment.

- Development of company-specific solutions to maintain or restore employability
- Support with workplace adaptation, implementation and qualification
Duration & Course
Once commissioned by the pension or health insurance provider, or another rehabilitation funding agency, participation is possible at any time. The individual support period is a maximum of six months. If reintegration into the labor market has not occurred after this period, we will prepare a personal final report with recommendations for future integration activities.
Financial support
Funding is provided through the responsible rehabilitation cost bearer.
